What made you choose a healthcare career?
I chose a Case Management career in healthcare because I have a passion for helping others and for making a positive impact in the community. As a Case Manager, the most rewarding social aspect of health care is the opportunity to directly impact and improve the lives of patients by connecting them with community resources and fostering supportive relationships with them and their families. Seeing patients regain independence, improve their quality of life, and achieve better health outcomes due to the collaborative efforts in social health care is what makes my job incredibly fulfilling. The impact of my role is evident through improved patient engagement, positive health outcomes and effective resource utilization.
How does the work you do at KCHC/Pillar Health make a difference in the lives of patients and our community?
My role as a Case Manager impacts patients by providing support and resources to navigate their healthcare needs while advocating for their social well-being and coordinating care. I excel in understanding and navigating the healthcare system to ensure patients receive optimal resources. My goal as a Case Manager is to build strong relationships with agencies which includes clear communication, mutual respect, and advocacy for patient needs. I prioritize transparency, collaboration, and maintaining current knowledge of available services to facilitate smooth referrals and comprehensive care coordination for each patient. This approach fosters trust and efficiency, ultimately benefiting patient outcomes. I’m passionate about making a positive difference and I’m grateful for the opportunity to serve our community.
